HIPAA SUMMARY

The security and privacy of user data is our top priority. We comply fully with the applicable provisions of the Security and Privacy rules, issued pursuant to the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191 ("HIPAA"), which sets forth guidelines for how to protect data from unauthorized access.

In order to comply with HIPAA, we have created specific policies and procedures, and have reviewed them with HIPAA experts for completeness and applicability. These policies range from login and password procedures to disaster recovery plans. We have made every effort to assure your data is safe and secure under our supervision, and review our practices on a regular basis to assure this in the future.

PASSWORD MANAGEMENT POLICY

This Password Management Policy has been published by Bend Surgery Center and we may amend or update this Policy from time to time if necessary or appropriate.

This Policy applies to both Users and Sharing Partners authorized to make use of bendsurger.com online bill payment services. The information available through the Services includes personal information about individual Users which is sensitive, confidential and protected by law.

Access to the Services is controlled by user names and passwords. Upon initial registration for use of the Services, bendsurgery.com generates a user name and initial password for each User. The user name and password is then presented whenever the User seeks to access the Services and protected information.

We must rely upon the validity of the user name and password in granting access to the Services and protected information, and cannot independently determine whether they are being presented by the User. Users therefore are required to protect their user names and passwords as follows:

Change the initial password provided generated when first signing on for use of the Services.

Change passwords no less frequently than every 90 (ninety) days.

Do not use actual words for the password. Do use passwords with mixed numbers, letters and symbols. Use passwords which are at least eight characters long.

Do not share the user name or password with any other person, or allow anyone else to use the user name or password

Do not let anyone else see, copy or steal the user name and password.

Notify us in writing at once if there is any reason to believe anyone may have used, learned, seen, copied or stolen the user name or password. Upon receiving and verifying such notice we will promptly terminate access under the compromised user name and password, and issue a new user name and password.

Failure to comply with these Password Management Requirements may lead to or cause the unauthorized disclosure of sensitive or confidential personal information, and may violate federal law.
